Some health plans are updating the administrative names they use for Community Alliance Medical Group (CAMG) physicians. These updates help ensure your coverage, referrals, and care process correctly in each plan’s system.

What You Need to Do

When applicable, health plans may list CAMG providers under the administrative contracting name LaSalle Health Plan Inc. Your doctors, your care team, and your services all remain part of Community Alliance Medical Group.

If your plan is listed below, you may be asked to confirm or update your medical group selection through your current health insurance plan. To continue receiving care from CAMG, the correct administrative medical group to select can be found using the Provider IDs as follows. Please contact your health insurance to update your medical group using the following Provider IDs:

Your insurance needs your doctor listed correctly to ensure smooth coverage and avoid misrouting. Provider ID updates shown on this page apply only to the health plans currently listed. Additional plans may update their records at different times. If your health plan requires a provider ID change, you will receive direct notification from CAMG once that information is confirmed.
